Unrestricted Bequest
“I hereby give and bequeath [____________ dollars / ____________ percent or all of the residue of my estate] to Beth Israel Deaconess Hospital–Needham ("BID Needham"), a Massachusetts charitable corporation, with an address of 148 Chestnut Street, Needham, Massachusetts, to be used for BID Needham’s general charitable purposes.”

Restricted Bequest
“I hereby give and bequeath [____________ dollars / ____________ percent or all of the residue of my estate] to Beth Israel Deaconess Hospital–Needham ("BID Needham"), a Massachusetts charitable corporation, with an address of 148 Chestnut Street, Needham, Massachusetts, for the benefit of BID Needham to be used for [specific purpose, program, department]. If at any time in the judgment of the trustees of BID Needham it is impossible or impracticable to carry out the designated purpose, they shall determine an alternative purpose as near as possible to my original intent.”
